The Empower Bundle

Supporting individuals in sustaining an active lifestyle while coping with chronic illnesses through the provision of education, advocacy, and resource management.


Objective-driven timeline of 1 to 8 weeks. This applies to postoperative surgical patients or those who have been recently discharged from a hospital or rehabilitation facility within the last 24 hours.

The Boost Bundle

The Wellness Bundle

Objective-driven timeline of 6, 8, or 12 weeks. Conduct an initial evaluation and create a tailored care plan in collaboration with your primary care physician or specialist. Provide educational resources for both the client and the caregiver.

Subscription Bundle

Continuous lifestyle management along with one specialized care item every two months. A 20% discount is available when the yearly subscription is paid in full.
